Life Map Order Form

Please use this form to place new orders for Life Map brochures.

Please review the following terms and conditions before placing your order:

  1. COST: The price for the new, updated, Fourth Edition of the OACB Life Map is $0.89 per booklet. This is only to cover printing expenses – OACB does not make a profit from the sale of Life Maps.

  2. BILLING:  No payment is due up front – all orders will be invoiced.  If you have questions about billing, please contact Lana Beddoes (lbeddoes@oacbdd.org) as soon as possible after placing your order.

  3. ORDER PICKUP/DELIVERY: Booklets will be mailed via US Postal Service. Shipping costs will also be billed to the boards and will vary for each order. Note: By default, Life Map orders are sent at the minimum possible delivery price to control costs. If you need your booklets sooner, please contact Lana Beddoes and specify in writing the speed of delivery you require.

  4. MINIMUM ORDER: The minimum order is 25 booklets.
